Barbiturates were the first group of drugs in the anti anxiety and hypnotic group. It has a wide range of drugs having different time periods of action. All of them act on the GABA receptor on the brain that is associated with wakefulness. It mainly produces dose dependent action starting from drowsiness, sedation, hypnosis, sleep, stupor, coma and death. On chronic dosage it produces alteration of the sleep architecture apart form the hang over symptoms. Progressively it produces deterioration in mental functions, affect cognition, increase daytime sleepiness and lack of concentration. Further over dosing will lead to depression of the central nervous system leading to death.

Barbiturates were dethroned by benzodiazepines which had all the desired effects and not the side effects. Yet barbiturates are widely abused though lesser than the other group of drugs. It comes in various street names like purple hearts, goof balls, downers, blue heavens, blue devils, rainbows, abbots, nembies, and so on.

It produces tolerance on chronic dosing which leads to dependence. Finally it leads to addiction and produces withdrawal symptoms on stopping. Sometimes people who are addicted may consume excess of these tablets due to automatism. And if not properly treated would lead to death of the person. Further barbiturate poisoning does not have any specific antidote.

The main problem in addiction is the loss of productivity of the person seriously affecting his social, economic and mental status. He may even indulge in anti social behavior for the sake of obtained drugs.

It is better to test for multiple tests at the same time rather than sticking on to the single drug testing. You cannot be sure that someone is addicted to just one drug!
